By logging into the Integrated Hospital Operations Monitoring System (iHOMS), you acknowledge and agree that you are an authorized employee of Democrito O. Plaza Memorial Hospital (DOPMH). All information you access, record, or process in this system is classified as confidential and is protected under Republic Act No. 10173 – Data Privacy Act of 2012. This includes, but is not limited to, sensitive personal and health information of patients, staff, and other hospital stakeholders.
As a DOPMH employee, you are required to:
• Access data solely for legitimate hospital duties within your assigned role.
• Protect all information from unauthorized access, disclosure, alteration, or destruction.
• Avoid sharing, copying, downloading, or transmitting any data outside official and authorized channels.
• Immediately report any suspected data breach to the Hospital Data Protection Officer (DPO).
Legal & Administrative Consequences
Any unauthorized access, use, or disclosure of information may lead to administrative sanctions, civil liabilities, and/or criminal penalties in accordance with the Data Privacy Act of 2012, other applicable laws, and existing hospital policies.
